Complete Registration Form (below)** - Pet & owner details - Referring veterinarian information - Medical records upload ###### **2. Medical Records Submission** Please request your pet's medical records from your veterinarian as soon as possible. Pre-Sedation Planning** COVE CT procedures will require a pre-sedation medication plan that you will need to give to your pet the morning of the procedure, before you check-in at COVE.   This medication protocol will be communicated to your regular veterinarian and you will need to work with them to obtain these medications in advance.  We appreciate your cooperation in making this process seamless.
